Off-line registration

Please choose from the available ordering options that best suits your needs:

Check / Money Order: Click here to go to the check order form. Fill out the applicable information, and send the form with payment to the address located at the top of the form. Please note that we can only accept checks or money orders in US Dollars drawn from a US bank.

Credit Card Order by Fax: Click here to go to the fax order form. Fill out the applicable information, and send the form to the fax number located at the top of the form.

Credit Card Order by Phone: To place your order by phone call us with your credit card information ready at one of the numbers below:

Purchase Order: We accept purchase orders from qualified corporations and organizations on orders of $50 or more. You can submit the purchase order to us by fax (801-497-9456) or mail (see address below.) We cannot accept purchase orders by email. Please note that we can only accept purchase orders with pricing in US Dollars.

Please include complete information including the product you wish to purchase, your billing address, phone number, etc. You MUST include an email address for product delivery.
